Marshall R. Posner is a leading head and neck oncologist whose research has reshaped treatment paradigms for oropharyngeal cancer, particularly in the era of human papillomavirus (HPV)-related disease. His major contributions center on de-escalation therapy—reducing treatment intensity to minimize long-term toxicity without compromising survival. As principal investigator of the landmark Sinai Robotic Surgery (SIRS) trials, he demonstrated that transoral robotic surgery (TORS) followed by reduced-dose adjuvant therapy can achieve excellent outcomes in HPV-positive patients, sparing them the debilitating side effects of high-dose chemoradiation. His work on the cost-effectiveness and quality-of-life utilities of TORS versus (chemo)radiotherapy has provided critical data for clinical decision-making, with two papers exceeding 100 citations each. Beyond surgery, Posner has pioneered personalized medicine using innovative platforms: a Drosophila (fruit fly) model identified a novel therapy for adenoid cystic carcinoma, and a robotics-based screen uncovered trametinib plus zoledronate as an effective combination for a KRAS-mutant colorectal cancer patient, leading to an 11-month response. His translational research also includes HPV-targeted immunotherapy, such as ADXS11-001, which enhances T-cell immunity prior to surgery.
