Antibiotic prophylaxis refers to the preventive administration of antimicrobial agents before, during, or after a medical procedure to reduce the risk of infection. Rather than treating an established infection, the goal is to suppress bacterial colonization at vulnerable sites — such as surgical wounds, the urinary tract, or implanted devices — before harmful populations can take hold. In urology and robotic surgery, prophylactic antibiotics are commonly used prior to procedures like radical prostatectomy, cystectomy, pyeloplasty, and in the long-term management of conditions such as vesicoureteral reflux in children, where recurrent urinary tract infections can cause progressive renal scarring. The practice matters because surgical site infections and catheter-associated urinary tract infections significantly increase patient morbidity, hospital stays, and healthcare costs. However, growing concerns about antimicrobial resistance have prompted ongoing debate about optimal drug selection, dosing duration, and whether single-dose regimens are as effective as multi-day courses. Tailoring prophylaxis using patient-specific antibiograms and evidence-based guidelines represents a critical strategy for balancing infection prevention against the broader public health imperative of antibiotic stewardship.
