With the recent development of robotic assistance, laparoscopic treatment of VUR has gained popularity. This study evaluates our initial experience with pediatric robotic-assisted laparoscopic intra- and extravesical ureteral reimplantation (RALUR) and compares outcomes with the open technique (OUR). METHODS A retrospective chart review was performed on all patients who underwent RALUR between March 2007 and March 2010 at our tertiary care center. Comparison was made with a case-matched cohort of patients who underwent OUR. Clinical success was defined as the absence of urinary tract infection; radiologic resolution was defined as the absence of any reflux on follow-up imaging. Groups were compared using t-test for numerical variables and Chi-Square Test or Fisher's Exact Test for categorical variables. A Kaplan-Meier model was used to compare success rates. Statistical significance was indicated by p < 0.05. RESULTS 39 patients (19 intra- and 20 extravesical) underwent RALUR during the study period. They were compared to 39 patients (22 intra- and 17 extravesical) who underwent OUR. Although both the intra- and extravesical RALUR approach were associated with a longer operative time (p < 0.001), children who underwent RALUR had shorter duration of urinary catheter drainage, less bladder spasm, and shorter hospital stays (p < 0.01). Overall success rates were similar among patients who underwent RALUR and OUR (p > 0.5). CONCLUSIONS RALUR offers similar success rates to the gold standard, OUR.
