The incremental cost-effectiveness ratio (ICER) is a standard health economic metric that quantifies the additional cost required to achieve one additional unit of health benefit — typically expressed as cost per quality-adjusted life year (QALY) gained — when comparing one intervention against another. In robotics and AI-assisted medicine, ICER is widely used to evaluate whether the higher upfront costs of robotic surgical systems, such as the da Vinci platform, are justified by improved patient outcomes relative to conventional laparoscopic or radiation-based alternatives. The calculation divides the difference in costs between two interventions by the difference in their health outcomes. ICER matters because robotic procedures often carry significantly greater equipment, maintenance, and operative costs, and decision-makers — including hospital administrators, payers, and policymakers — rely on ICER thresholds to determine whether adopting robotic technology represents good value. By framing adoption decisions in rigorous economic terms, ICER analysis helps balance clinical innovation with sustainable healthcare resource allocation.
