摘要

Introduction and Objectives: Laparoscopic
\npartial nephrectomy is a technically
\nchallenging procedure and important limitations
\nwith conventional laparoscopy
\ninclude spatial orientation and depth perception.
\nThree-dimensional (3D) video
\ndisplay has been introduced with robotic
\nsurgery, however the higher initial investment
\nand associated running costs accompanying
\nrobotic technology makes it not
\ncost-effective in certain centres. We developed
\na pilot study to compare the clinical
\noutcomes of 3D laparoscopic partial
\nnephrectomy with conventional twodimensional
\n(2D) laparoscopic partial
\nnephrectomy by a single surgeon in the
\nACT. The aim of this study was to assess
\nwhether 3D nephron-sparing laparoscopic
\nsurgery has better intraoperative and oncological
\noutcomes compared to conventional
\n2D nephron-sparing laparoscopic
\nsurgery.
\nMethods: Retrospective review of all consecutive
\npatients who underwent laparoscopic
\npartial nephrectomy since the
\nintroduction of a 3D video system at
\nNational Capital Private Hospital in 2012
\nunder a single surgeon (HH). Clinical outcomes
\nmeasured were the length of hospital
\nstay, total operative time, blood transfusion,
\npositive surgical margins and tumour margin
\nclearance. These outcomes were measured
\nfor those patients that underwent 3D
\nlaparoscopic partial nephrectomy (LPN)
\nand compared to historical data by the

\nsame surgeon for patients that underwent
\nconventional 2D LPN over the preceding
\n4 years. Statistical analysis of the data with
\nchi-square and Fisher’s exact test was performed
\nwith SPSS v.23.0.
\nResults: 22 patients were included in 3D
\nlaparoscopic partial nephrectomy (LPN)
\ncohort with 17 patients included in the 2D
\nLPN cohort.
\nConclusions: 3D laparoscopic partial
\nnephrectomy is non-inferior to conventional
\n2D laparoscopic partial nephrectomy
\nin our pilot study. The application
\nof 3D technology in a training setting to
\ndecrease the learning curve for inexperienced
\nlaparoscopic surgeons is worth
\ninvestigating.
